Case Overview
Lydia was diagnosed with left breast DCIS at 47 years old. She underwent bilateral mastectomies for definitive breast cancer management and risk reduction and DIEP flap breast reconstruction.

Pre-operative before bilateral Wise pattern mastectomies
Her breasts have skin-on-skin contact and her nipples are low, supporting a mastectomy nipple-taking mastectomy with extensive skin excision. Her mastectomies were performed through a Wise pattern (inverted T) incision 10 days before DIEP flap breast reconstruction. After pathology confirmed negative margins, she underwent DIEP flap surgery.
Eight weeks after bilateral DIEP flap breast reconstruction
There is a triangle of abdominal skin visible in the vertical closures on both sides. This was used for monitoring in the immediate post-operative period, but it was eventually removed. All scars are pink and raised as part of the early, ongoing healing process.
Five months after DIEP flaps and six weeks after right and left breast revisions
Lydia’s revisions included excision and closure of the visible abdominal skin island and fat grafting. Fat grafting was used to fill in visible scoops and hollows between the DIEP flap and surrounding tissues.
Eight months after DIEP flaps and four weeks after right and left nipple reconstruction.
In addition to nipple areola reconstruction, the patient chose to have cosmetic liposuction on her right and left sides as part of the same procedure through a prone (face down) position during nipple reconstruction.
